Timely access to healthcare is a major issue for rural residents. This is particularly relevant to Mississippi as the majority (65) of Mississippi's 82 counties are rural[1] and often citizens of these communities have limited access to healthcare and rely upon a single hospital. The primary goal of this research initiative is to accurately assess health and economic outcomes among rural communities in Mississippi. Projects resulting from this initiative will inform current strategies to improve access to healthcare in rural communities and include the following three objectives:Develop the capacity and expand collaboration efforts with academic and community-based partners to advance research ideas and apply for funding that supports and promotes research (e.g. cost, outcomes, and quality) related to access to healthcare in rural communities Design and conduct empirical and applied research, and/or evaluation projects that estimate both the economic and health impacts of healthcare in Mississippi. Goals / Objectives
The primary goal of this research initiative is to accurately assess health and economic outcomes among rural communities in Mississippi. This initiative will acommplish the follwing three objectives:OBJECTIVE 1: Develop the capacity and expand collaboration efforts with partners to advance research ideas and apply for funding that supports and promotes investigation of research questions (e.g. cost, outcomes, and quality) related to access to healthcare in rural communities OBJECTIVE 2: Design and conduct empirical and applied research, and/or evaluation projects that assess how access to healthcare leads to both health and economic impacts in Mississippi.OBJECTIVE 3: Disseminate data and reports of the research results that illustrates the potential of access to healthcare as health, costs, and policy effects among communities in MS.
Project Methods
OBJECTIVE 1: Develop the capacity and expand collaboration efforts with partners to advance research ideas and apply for funding that supports and promotes research (e.g. cost, outcomes, and quality) related to access to healthcare in rural communities Working plans/methods: Dr. McDoom will continue to work with existing partners (e.g. University of Memphis, Brandeis University, University of Arkansas, Mississippi Hospital Association, Harvard School of Public Health, Harvard Law School,) to advance existing projects and identify new projects and funding sources. She will also work with other stakeholders, such as the MS Department of Health, to develop additional partnerships.OBJECTIVE 2: Design and conduct empirical and applied research, and/or evaluation projects that assess how access to healthcare leads to both health and economic impacts in Mississippi. Working plans/methods: Research, education, and evaluation may involve either primary or secondary data collection or analyses. Primary data collection may include telephone surveys of rural hospital administrators or county health officials conducted at the Social Science Research Center or through the Mississippi Hospital Association. Focus groups and interviews may be alternative data collection methods. Secondary data analysis of publicly available data sets (e.g. American Community Survey, American Hospital Association Data, Area Resource File, etc.) could be used to explore research and evaluation questions that are relevant to access to healthcare in rural communities. These secondary data sources could be linked together to conduct multi-level analyses at the county and hospital level. Potential types of analyses that could be used include: GIS mapping and modeling of time to nearest health facilities, mixed effects models including both fixed effects (county level) with random effects (hospital level), and economic impact (changes in revenue, profits, wages, employment, etc.).OBJECTIVE 3: Disseminate data and reports of the research results that illustrates the potential of access to healthcare as health, costs, and policy effects among communities in MS. Working plans/methods: Dr. McDoom will continue efforts with existing partners to identify ways to effectively disseminate the results and implications from the initiative. She will continue to work with existing partners and identify other stakeholders with a vested interest in access to healthcare in rural settings. She will also disseminate results and findings at local, regional, and national conferences related to access to healthcare and rural health.

Target Audience:During this reporting period I engaged with a variety of target audiences via in-person meetings including: The Mississippi Hospital Association, hospital administrators at University of Mississippi Medical Center, health policy decision makers (including federal and state legislators), researchers who are interested rural health and access to health care. My efforts during this reporting period includes: brief presentation at research unit monthly meeting; dissemination of completed report to key stakeholders (University of Memphis, Center for Mississippi Health Policy, and Harvard University) Changes/Problems: Nothing Reported What opportunities for training and professional development has the project provided?The undergraduate student who works on this project has received both research training, enhanced writing and analytical skills, and professional development through her involvement with this project. t. Her skills were enhanced through training on how to conduct literature reviews, how to manage references through a reference manager software, statistical software training on how to use SPSS and Microsoft Excel. How have the results been disseminated to communities of interest? Nothing Reported What do you plan to do during the next reporting period to accomplish the goals?During the next project period we plan to continue our dissemination efforts including press releases, speaking to stakeholder audiences, and public release of our report. We will also plan to prepare a manuscript for peer-review publication.

We successfully applied for funding from the Center for Mississippi Health Policy and were able to apply those funds to examine the economic implications of rural hospitals potentially closing. -We were also able to use the knowledge and findings from this project as an invited oral presenter on a panel regarding "The State of Rural Health Care in the Mississippi Delta" at the Delta Regional Forum: Population, Development, and Entrepreneurial Problem Solving, July 15-17, 2015 in Clarksdale, MS. - Additionally, using the knowledge and findings from this project, I served as an expert liaison and gave critical feedback to several federal proposals that were developed to enhance the stability of rural hospitals. OBJECTIVE 2: Design and conduct empirical and applied research, and/or evaluation projects that assess how access to health care leads to both health and economic impacts in Mississippi. - We have completed the main analysis for this project and have examined both the health and economic implications at a county and state-wide level. OBJECTIVE 3: Disseminate data and reports of the research results that illustrate the potential of access to healthcare as health, costs, and policy effects among communities in MS. - We have not yet begun our dissemination phase, but have completed our report. Nothing Reported How have the results been disseminated to communities of interest? Nothing Reported What do you plan to do during the next reporting period to accomplish the goals? I plan to make significant progress on completing the data analysis and plan on presenting our findings in the spring or summer of 2015 (via conferences and selected talks). I also hope to continue building this resesarch intitative by seeking additional funding to examine other issues related to access to healthcare. Futhermore, we will prepare and submit at least 1 manuscript that describes our findings.

Impacts
What was accomplished under these goals? During the reporting period (September 10-30, 2014) signficant progress was made on the empirical research related to the impact of rural hospital closure in Mississippi. We are currently analyzing data which we purchased from the American Hospital Association in September 2014. I have formed a collaborative working group with resesarchers and economists at the University of Memphis to examine the data and to also collaborate for future projects. .
